BASIC LIFE AND ACCIDENTAL DEATH AND DISMEMBERMENT (AD&D)

At PharmaLogic, your wellbeing—and your family’s—is at the heart of our benefits. That’s why you automatically receive Basic Life and AD&D insurance at no cost as soon as you join the team.

Life insurance provides financial support to the beneficiary you choose, while AD&D offers the same protection if an accident results in death or a covered serious injury. It’s peace of mind for you and the people who matter most.

Open Enrollment is the perfect time to review your beneficiary information!

VOLUNTARY LIFE AND AD&D
 

You have the opportunity to add extra financial protection through Voluntary Life and AD&D coverage. You can also choose to cover your spouse and/or children, as long as you elect coverage for yourself. You can find the rates in ADP and are determined by your age, and the benefit amount you select.

During your first enrollment window, you can choose up to the guaranteed issue amount with no medical questions asked. If you enroll later—or request a higher amount—Evidence of Insurability (EOI) may be required. EOI is a brief health form the insurance carrier uses to review your request.

Short and Long Term Disability

Short-Term Disability (STD)

Short Term Disability coverage is provided at no cost to you—PharmaLogic pays the full premium. Since the company funds the benefit, STD payments are taxable.
If you’re unable to work because of a non-work-related illness or injury, Short-Term Disability (STD) helps support your income while you recover. STD pays 60% of your weekly base earnings, up to $2,000 per week, for up to 12 weeks.

Long-Term Disability (LTD)

*NEW COMPANY PAID BENEFIT*
We’re excited to share that PharmaLogic now covers the full cost of your Long-Term Disability (LTD) insurance. Since the company pays for this benefit, any LTD payments you might receive from a covered disability would be tax-free, giving you added peace of mind during a challenging time.

LTD provides essential income protection if you are unable to work due to a long-term illness or injury.

COORDINATION OF DISABILITY BENEFITS

Your benefit may be reduced if you receive disability benefits from retirement, Social Security, workers’ compensation, state disability insurance, no-fault benefits or return-to-work earnings. Contact HR for more information.
