Medical Options to Meet You and Your Family's Needs

Our three medical plans offer you choice to meet your individual needs.  Choose from our Employer Funded HRA that covers the first dollar costs for the plan, an HSA option with company seed money you decide when and where to use and an OAP plan where you will have fixed office visit costs.

Employee Funding Plans

  • Employer Funded HRA - PharmaLogic will apply HRA Funds BEFORE you pay any out-of-pocket costs for doctor visits, hospitalizations, prescriptions, and other care.

  • You decide where and when to spend the money.  You must open up the HSA account to receive the seed money.

  • HSA Plan provides $1,500/$3,000 PharmaLogic contribution to your HSA account. This amount is prorated based on your benefits effective date. 

  • The Employer contributions decrease by

$150 per month Employee Only

$300 per month Family

  • Example:

July 1st effective date= $900 Employee Only/ $1,600 Family

December 1st effective date = $150 Employee Only/ $300 Family

ID Cards - Create an account or log into myCigna.com

1. Click ID Cards at the top right ( if on the mobile app, it will be on the bottom left corner)

             All current ID Cards for the person logged in populate first. To request a card for                       another family member, click the drop-down. 

2. Click on the blue View< Plan Type<ID Card button on the type of card that you need                        An image of the card will populate.

              To submit a request for a physical copy of the card, click the link under the card                       image. 

3. Click Download to download the image or Print to print a paper copy of your card.

              On the myCigna mobile app, click share to email the image, as needed. You can                     also save a copy to your Apple or Google wallet.
